STEP-DOWN MODEL

At Life Skills Hub, our primary goal is to support young people to socially connect and become more independent. We understand that the path to independence is a journey – and it is important to provide young people with the right opportunities to match their skills and interests.  

Our services can be broken down into 4 categories: 

  • 1:1 Therapy Services 

  • Therapy Groups 

  • Structured Groups 

  • Recreational Groups 

 

We adopt a ‘step down’ approach – where clients can transition through a variety of services in working their way to independence. All services work towards identified goals and are developed with particular skills in mind.

It is important to note that clients may engage in a variety of services across different categories to address different goals.  

1:1 THERAPY

  • Therapy sessions with an Allied Health Professional.

  • Addressing development of specific skillsets required to engage in groups.

  • Specific and individualised sessions targeting outlined therapy goals.

THERAPY GROUPS

  • All programs designed/co-designed by an Allied Health Professional.

  • Programs are facilitated by an Allied Health Professional or Level 2 Allied Health Assistant.

  • Provides opportunity for young people to connect with peers while addressing similar therapeutic goals.

  • Facilitation of increased social engagement and positive interactions with peers.

STRUCTURED GROUPS

  • All programs are co-designed by an Allied Health Professional.

  • Programs are facilitated by an Allied Health Assistant or Mentor.

  • Addresses broader goals better suited to a group environment. For example, goals targeting social engagement, confidence, leisure and increased community participation.

  • Development of skills during activities that are of interest to the young person.

  • Facilitation of increased social engagement and positive interactions with peers.

MENTORING & 

RECREATIONAL GROUPS

Mentoring

  • 1:1 Services

  • Continuing to develop independence with regards to set goals, but with reduced support.

  • Focus on developing trust and support where needed, however promoting increased independence.

Recreational Groups​

  • Larger group sizes

  • Primary goal is to facilitate increased community participation and social engagement.

  • Promote positive leisure activities and development of friendships.
